How CCHN Works
Over your lifetime each provider you visit for care holds a piece of your medical history. Through Health Information Exchange services such as that provided by CCHN, a more complete picture of your health and medical history comes together for your providers, allowing them to work with you to make a better, more-informed decision about your care.
Effective, accurate, secure, and up-to-date records that follow you wherever your healthcare journey takes you improves your quality of care, and the continuity of your care. These services give you better access to personal health information, and reduce unnecessary and duplicative testing. The result? Better care for you as a patient, no matter where you are.
Of course, that is only if you want.
Coordinated Care Health Network provides you as a patient the option to change your participation in CCHN, … meaning you can tell us not to share your record. NOTE: Opting out means participating physicians and authorized healthcare providers will not be able to access your personal health records through Coordinated Care Health Network.
Any patient can change their participation to share or not share health information with Coordinated Care Health Network at any time. Simply fill out the downloadable PDF form and mail it to our offices at the address below and your record will be updated.
A note about HIPPA and Your Privacy — CCHN uses a secure cloud-based computer network. The information exchanged is used only by authorized personnel, and is protected by state and federal laws. Participation in CCHN is voluntary.
